Supported Living Program Overview
The Supported Living Program helps adults with physical and developmental disabilities live more independently in their own homes and apartments throughout Southeastern Massachusetts. The program provides Case Managers who help consumers manage their affairs and their Personal Care Attendants (PCAs). Options is an approved provider to serve consumers referred by MRC, DDS, ABI, MFP.
In order to be eligible for the Supported Living Program, an individual must have:
- A severe physical disability as well as a sensory, cognitive, or emotional limitation
- Have a mobility impairment which results in the need for adaptive equipment
- Be unable to live independently without supported living services
- Be able to benefit from supported living services
- Be at least 18 years old or be determined eligible by MRC for the Turning 22 program.
Options provides Supported Living Case Management and/or Surrogacy to people who use the MassHealth PCA program or to people who need ongoing support in the areas household management, social/recreational, personal health care, transportation, financial, and self-advocacy. Case Management/Surrogacy also assist with PCA Management to include being very involved with the process of hiring, training and firing PCAs. Case Manager/Surrogacy offers support to the person living in the community in his/her own residence to gather all necessary information in order to make self-determined choices.
Options was recently approved for MassHealth Adult Foster Care. This program is designed for people who want a little extra support given by a live in caregiver, or a caregiver to live with. Families are assessed and careful matching takes place. Caregivers are responsible to be mature with good judgment and have the time to devote to the care of an individual who needs cuing or supervision in an ADL, or hands on personal care. Consumers are referred through any means.
Options provides Individual Supports and Community Rehabilitation through the Acquired Brain Injury Waiver.
