Narrative

The Community Living program provides care management, direct care, and referral services to individuals with disabilities that enables them to live in the community with dignity.

A variety of housing and support models are available, including:

  • Adult Family Care / Home Share

  • Continuous Resident Support Program
  • Supported Living Outreach 
  • In-Home Supports
