Narrative

The Community Living program provides case management, direct care, and referral services to individuals with disabilities that enables them to live in the community with dignity.

A variety of housing and support models are available, including:

  • Adult Family Care / Home Share
  • Self-Directed Household
  • Congregate Housing
  • Continuous Resident Support Program
  • Supported Living Outreach 
  • In-Home Supports
  • Outreach and 24 hour Residence for Co-occurring Disorders: Substance Use and Mental illness

In addition to providing care supports, Community Living provides participants with a number of opportunities for personal growth, community participation and networking with each other. Some of these opportunities have included:

  • Art classes
  • Writing groups
  • Health & fitness activities
  • Monthly Wii fitness groups
  • Monthly lunches
  • Day trips
  • Annual picnics
  • Community volunteer work
