Narrative
The Community Living program provides case management, direct care, and referral services to individuals with disabilities that enables them to live in the community with dignity.
A variety of housing and support models are available, including:
- Adult Family Care / Home Share
- Self-Directed Household
- Congregate Housing
- Continuous Resident Support Program
- Supported Living Outreach
- In-Home Supports
- Outreach and 24 hour Residence for Co-occurring Disorders: Substance Use and Mental illness
In addition to providing care supports, Community Living provides participants with a number of opportunities for personal growth, community participation and networking with each other. Some of these opportunities have included:
- Art classes
- Writing groups
- Health & fitness activities
- Monthly Wii fitness groups
- Monthly lunches
- Day trips
- Annual picnics
- Community volunteer work
